Psycho-cardiology is a subspecialty of Psychosomatic Medicine, constituting a major research focus in our department. Our research group is part of the Heart Center Göttingen, the Heart Research Center Göttingen and the German Center for Cardiovascular Research. Main research topics include bidirectional mind-body-interactions in cardiovascular diseases and psychosocial treatments (including, eg, psychotherapy) for patients with heart disease. A research laboratory for molecular psycho-cardiology has been launched early in 2011.

Current core areas of research:

  • Multicenter trial on a “Stepwise Psychotherapy Intervention to Reduce Risk in Coronary Artery Disease” (SPIRR-CAD; funded by the German Research Fund, 2008-2013).
  • Development and evaluation of a telephone-based peer-counselling program for depressed women with coronary heart disease (“women's hearts telephone”, in collaboration with KKH-Allianz, 2010-2012)
  • Studies of psychological well-being, personality quality of life, psycho-physiology and prognosis ind patients with cardiovascular diseases or risk factors (as part of local, national and international research consortia, eg, competence Network Heart Failure, Aldo-DHF, Ex-DHF, CIBIS-ELD studies, etc., funded by the German Ministry for Education and research and others)
  • Molecular research on psychocardiological interactions (Research Laboratory)
  • Collaboration in expert panelsand guideline committees in cardiovascular medicine.

Randomized controlled trail of psychodynamic psychotherapy and cognitive therapy for adolescent girls with bulimia nervosa and atypical bulimia nervosa

  • In our study design, we compare psychodynamic psychotherapy and cognitive therapy in adolescent females suffering from bulimia nervosa or atypical bulimia. Both treatments are manualised and conducted with maximum 60 therapy sessions. We hypothesize that both treatment conditions are effective, however differential therapy effects, due to structural characteristics, are expected. To asses the severeness of bulimia nervosa we use the Eating Disorder Examination as questionnaire and interview as well as the Eating Disorder Inventory 2 . We also use the Symptom Check List (SCL-90-R, German version: Franke 2002) to capture the overall severity of psychiatric symptoms. To check for comorbidity, we use the Structured Clinical Interview for DSM Disorders (SCID I). To measure self-conscious affects, the German version of the Test of Self-Conscious Affect (TOSCA-A, Tangney et al. 1991) for adolescents (TESE-KJ, Kronmüller et al. 2008) is used. For multiaxial psychodynamic diagnosis we use the operationalized psychodynamic diagnostics system for children and adolescent (OPD-KJ).
  • We intend to treat one hundred adolescent females suffering from bulimia nervosa or atypical bulimia were treated with either psychodynamic psychotherapy or cognitive therapy.
  • Patients are found through public advertising on the website of our ambulances. Measurements are carried through at Baseline, the 30. Session, end of therapy (60 sessions) and at 1-year follow-up.
  • The study protocol was approved by the local ethics committees in Göttingen and Heidelberg and was prepared in accordance with the ethical standards laid down in the Declaration of Helsinki.

Clinical research of value commitment and the notion of the pathic self:

Clinical research of value commitment is a new field of study within psychodynamically oriented psychotherapeutic research. This new field investigates the hitherto unconsidered significance of moral experience for the process and effectiveness of psychodynamic psychotherapy. The methodical approach adopted by the project includes theories from social science and philosophy.

The conceptualized clinical research of value commitment is divided into (1) a theoretical part and (2) an empirical part.

(1) The theoretical part is about the development of a clinically applicable and normative contextualized concept of the self. In the past 30 years the research on the self in different theoretical and empirical disciplines has generated a lot of new findings, considerations and theories: “One of the insights that has emerged from this ongoing discussion is the recognition that the self is so multifaceted a phenomenon that various complementary accounts must be integrated if we are to do justice to its complexity “ (D. Zahavi 2010). The multiple facets of the self are expressed in multiple self-descriptions: emergent self, core self, subjective self, verbal self (D. Stern 1985); ecological self, interpersonal self, extended self, private self, conceptual self (U. Neisser 1988); protoself, core self, autobiographical self (A. Damasio 1999); minimal self, narrative self (S. Gallagher 2000); embodied self (Fuchs et al 2010).

The term pathic self is supposed to present a facet that has not been described so far. Based on Victor v. Weizsäcker's considerations about a pathic anthropology , the conceptualization of the term pathic self is about the articulation of normative aspects within a clinically applicable theory of the self.

Such a normative contextualized theory of the self needs a sound basis that applies psychogenetic and moralgenetic aspects to each other. Drawing on the mentalization model posited by Fonagy et al. (2002) and the theory of the self advocated by C. Taylor (1989) differences between a recent developmental psychological theory and a recent philosophical theory of the self are presented and then critically applied to one another.

Secondly, the theories are converged to provide a framework for the development of a normative contextualized concept of the self, using the guideline of affects ( am Leitfaden der Affekte) .

(2) The terms value commitment narrative (Wertbindungsnarrativ) and narrative evaluative scene (narrativ-evaluative Szene) are focused in the empirical part: in these terms the results of the hitherto uncorrelated narrative ethics and clinical narrative research are applied to each other.

The term narrative ethics includes approaches from different disciplines (philosophy, theology, literary studies, bioethics). These approaches describe the correlations between the act of narrating and the reflection, modification and communication of value commitments. In addition, these correlations are used as a starting point for a reflection on morals. This is neither done by illustrating previous discursively achieved abstract ethical considerations and principles in a narrative way only, nor by entirely transforming argumentation and discursivity into narration (Lesch 2003), but by examining, “in what way narrative, which is never ethically neutral, proves to be the first laboratory of moral judgement.” (Ricoeur). Generally speaking, narrative ethics are about the phenomenons of morals, which are constituted in and through narrating. Narrative ethics also examine how and in what way moral insights and commitments develop during the act of narrating and in the reception of narrations, which could not be developed in the same way through argumentation or discursivity.

Within the psychodynamically oriented clinical narrative research , different approaches have been established during the past years: Luborsky‘s (1977) Core Conflictual Relationship Theme method (CCRT); Strupp and Binder's (1984) Dynamic Focus as a focal narrative; Boothe's (since 1994) narrative-analytic program called JAKOB; Main & Goldwyn's (1985-1996) Adult-Attachment-Interview; T. Habermas' (since 2000) research on emotion representation and regulation by narratives.

Aspects of both research directions, especially of Boothe's, T. Habermas' and Ricoeur's concepts are connected in the terms value commitment narrative and narrative evaluative scene . Analyzing patients' narrations, a more detailed empirical understanding of correlations between affectivity and normativity is supposed to be gained by looking at the narrative enactment and modelling in the clinical process.
